How the transfer workflow works

Collect the record details

Start with the patient name, date of birth, medication name, strength, Rx number when available, and the current pharmacy contact information.

Confirm refill and prescriber status

The team may need to verify refills, reach out to the prescriber, or request a new prescription if the original record cannot be transferred as-is.

Route to the right next step

When the transfer is complete, the request moves into the pharmacy's normal review and fulfillment workflow. If it cannot be transferred, the patient is guided toward the next documented option.

Details that help avoid delays

Current pharmacy information

Provide the name, location, and phone number of the current pharmacy so the transfer request can be initiated efficiently.

Prescriber details

Prescriber name and phone number help when refills need confirmation or a new prescription becomes necessary.

After-hours note

Your On Call, LLC instructions document emergency support for after-hours needs, but also state that CII scripts cannot be filled through that pathway. Transfer requests that fall into restricted categories may require next-business-day follow-up.
